Will a rim from a 2004 Mach I fit on a 1968 mustang with 5 lug pattern?  What mods would be required?

On all 5 lug Mustangs the lug pattern is the same. Your problem will be backspacing and offset.
If you purchase 1.25" spacers your '68 the 04 wheels will bolt right on. There is a guy in the classic section that has a thread on this.....

I don't think they will really fit with just spacers but I could be wrong.  There's a special kit ($250) that can be purchased to make '04 Bullits or Mach 1s fit 67-68s.  That's what I plan on using.  Check Mustang Monthly How To's.  They have 2 really good articles on the subject.
